To learn more, visit www.habitathealth.com.</div>Estimated Start Date: 11/2/2026  Role Scope: The Clinical Services Manager (CSM) is a key leadership role within the Habitat Health PACE Center, responsible for overseeing the nursing and clinic teams to ensure high‑quality, participant‑centered care. Reporting to the Center Executive Director, the Clinical Services Manager oversees clinic operations and outcomes while providing guidance and support to RNs in their case management responsibilities and their work as members of the interdisciplinary team (IDT). This role partners closely with the Center Manager/Director to ensure the overall success of the Center. Depending on Center census and structure, the Clinical Services Manager may also oversee additional members of the IDT.  Core Responsibilities & Expectations for the Role  <ul> <li>Provide clinical leadership and oversight to members of the Interdisciplinary Team (IDT) and clinic staff, ensuring high‑quality, participant‑centered care. </li> <li>Ensure clinical competency of direct reports through skills checks, coaching, mentoring, performance management, and ongoing professional development. </li> <li>Lead onboarding, training, and continuing education for nursing and clinic team members to maintain high standards of care and regulatory compliance. </li> <li>Monitor and ensure adherence to clinic and IDT service level agreements (SLAs), regulatory timelines, and documentation requirements. </li> <li>Oversee care transitions across settings (hospital, skilled nursing facility, home, etc.), coordinating with the clinical team, Medical Director, and other center leaders to ensure continuity and safety. </li> <li>Drive achievement of key performance indicators (KPIs) through effective planning, workflow optimization, participation in weekly tactical and monthly operating reviews, and provide support with quality initiatives for the Center. </li> <li>Maintain compliance with PACE, DHCS, and CMS regulations for audit readiness of the clinic, med room, and lab room; including audit preparation, monitoring areas of non‑compliance, and implementing corrective action plans. </li> <li>Support clinic operations by addressing staffing needs, workflow issues, and operational barriers, escalating when appropriate. </li> <li>Provide direct clinical care as needed, including triage, emergency response, wound care, medication and vaccine administration, home care support, and participant education. </li> <li>Perform other related duties as assigned to ensure smooth clinic and Center operations. </li> </ul> Required Qualifications:   <ul> <li>Active and unrestricted Registered Nurse (RN) license in California. </li> <li>3+ years of clinical leadership experience, with demonstrated ability to lead teams and drive performance across multiple workflows. </li> <li>Proven ability to supervise and develop clinical staff, including nurses, medical assistants, and frontline supervisors. </li> <li>Strong decision‑making and prioritization skills, with experience implementing scalable processes in a fast‑paced clinical environment. </li> <li>Advanced communication and relationship‑building skills, with the ability to build trust, foster accountability, and collaborate effectively across interdisciplinary teams. </li> <li>Experience working with complex geriatric populations, including individuals with multiple chronic conditions, functional limitations, and behavioral health needs. </li> <li>Knowledge of clinical quality standards, regulatory requirements, and compliance expectations relevant to PACE or similar value‑based care models. </li> <li>Certified in CPR/BLS. </li> <li>Aligns with our purpose and our values, and is excited about living those out in daily practice </li> <li>Ability to thrive in a fast-paced, evolving environment with comfort navigating ambiguity, adapting quickly, and contributing to continuous improvement </li> <li>Strong learning and growth mindset, including seeking feedback, engaging in healthy debate and using data and curiosity to inform decisions. </li> <li>Acts with integrity and ownership, considering the broader organizational impact, doing the right thing, and following through to deliver results. </li> </ul> Nice to have:  <ul> <li>Bachelor’s Degree in relevant field </li> <li>Experience supporting or overseeing care coordination, case management, or interdisciplinary team (IDT) processes preferred. </li> <li>Bilingual Spanish/English or Cantonese/English preferred, other language abilities are also a plus</li> </ul> Essential Functions & Physical Requirements  The following statements describe the general nature of work and physical expectations common across roles at Habitat Health. The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.  In addition, this position is eligible for a comprehensive benefits package, including medical, dental, and vision insurance; paid parental leave, short- and long-term disability coverage; life insurance; flexible spending accounts; a 401(k) savings plan that includes a match; paid time off; and company paid holidays.  At-Will Employment   Employment with Habitat Health is at-will.
