To learn more, visit www.habitathealth.com.</div>Estimated Start Date:  11/30/2026 Role Scope: The PCA Supervisor leads and directly oversees the Personal Care Attendant team at the PACE center, ensuring participants receive safe, dignified, high-quality assistance with activities of daily living (ADLs). This role is responsible for day-to-day team supervision, training and competency oversight, and regulatory compliance for direct participant care staff, working in close partnership with the Interdisciplinary Team (IDT), nursing leadership, and the Center Director.  Core Responsibilities & Expectations for the Role  <ul> <li>Supervise, schedule, and coordinate daily assignments for the PCA team to ensure adequate participant coverage at the day center, including meal service and daily activities and for external medical appointments. </li> <li>Serve as the first point of escalation for PCA staff questions, participant concerns, and day-to-day operational issues. </li> <li>Ensures PCA-maintained logs are documented timely and accurately in accordance with facility processes. </li> <li>Manage and oversee participant incontinence supply delivery process and monitor day center inventory stock. </li> <li>Ensures timely completion of all PCA staff documentation, including ADL support (toileting, bathing), progress notes, grievances and SDR’s aligned with regulatory timeline requirements. </li> <li>Conduct regular check-ins, team huddles, and one-on-ones to support staff performance and morale. </li> <li>Conduct performance evaluations, coaching, and corrective action for PCA staff as needed. </li> <li>Coordinate ongoing (not just initial) competency evaluations. </li> <li>Monitor the quality and consistency of ADL support delivered by the PCA team against each participant's individualized care plan. </li> <li>Communicate participant status changes, incidents, or care plan concerns to the IDT and nursing leadership in a timely manner. </li> <li>Support the IDT in adjusting care plans based on PCA observations of participant needs. </li> <li>Support the team of PCAs in delivering daily care, as needed. </li> </ul> Required Qualifications:   <ul> <li>Certified Nursing Assistant (CNA) certification from accredited program. </li> <li>Associates degree or at least 2 years of experience in a supervisory, lead, or preceptor role. </li> <li>Active Food Handlers Certification </li> <li>Demonstrated knowledge of ADL support, infection control, and safe patient handling </li> <li>Strong communication skills; ability to interface with IDT, nursing, families, and participants. </li> <li>Experience overseeing daily workflows and supporting team performance through coaching and feedback. </li> <li>Ability to make routine operational decisions, solve problems in real time, and maintain quality standards. </li> <li>Demonstrated ability to manage schedules, prioritize work, and keep operations running smoothly. </li> <li>Aligns with our purpose and our values, and is excited about living those out in daily practice </li> <li>Ability to thrive in a fast-paced, evolving environment with comfort navigating ambiguity, adapting quickly, and contributing to continuous improvement </li> <li>Strong learning and growth mindset, including seeking feedback, engaging in healthy debate and using data and curiosity to inform decisions. </li> <li>Acts with integrity and ownership, considering the broader organizational impact, doing the right thing, and following through to deliver results. </li> </ul> Nice to have:  <ul> <li>Bilingual: Spanish/Mandarin/Cantonese preferred. </li> <li>Experience in a PACE setting or working with older adults </li> </ul> Essential Functions & Physical Requirements: The following statements describe the general nature of work and physical expectations common across roles at Habitat Health. This position is also eligible for a target annual bonus of 5% and may be eligible for additional incentive compensation, as applicable. The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.
