About the Role

Case Management involves the IEP/504 creation&nbsp;process.<br>● Provides social assessments and reports as needed for students receiving Initial evaluations or Reevaluations.</p> <p>●&nbsp;Conduct &amp; Coordinate assessments and evaluations to determine the needs of individual students.<br>● Conduct meetings during school hours.<br>● Supports the coordination of SEMI reporting and related services.<br>● Attend Assessment Plan meetings for all students referred for Initial evaluation<br>● As assigned, completes social assessments, creates reports submitted to the Child Study&nbsp;Team Manager per Achievers ECP procedures<br>● Maintain a caseload in compliance with state timelines and other mandated state/school&nbsp;guidance.<br>● Complete Case Management responsibilities as assigned<br>● Attend Eligibility Conference for Initial and Re-evaluations if requested<br>● Work in compliance with the Catapult Learning Policy and Procedure manual for Evaluation;&nbsp;Determination services in New Jersey<br>● Expert knowledge in NJ Code and procedures regarding student services and&nbsp;accommodations<br>● Counsel students with IEPs<br>● Support attendance team<br>● Screen for signs of trauma and counsels students with trauma and other mental health needs<br>● Other responsibilities as assigned<br>● Work closely with school administration, teachers, and other staff members to&nbsp;identify students who need additional support.<br>● Develop and implement Individualized Education Plans (IEPs) for students with&nbsp;special needs.<br>● Provide support to students with behavioral, emotional, and academic challenges.<br>● Facilitate communication between school staff, families, and community resources.<br>● Provide crisis intervention and support to students and families as needed<br>● Maintain accurate and up-to-date records of student progress and services provided<br>● Attend meetings and trainings as required</p> <p>&nbsp;</p> <p><strong>Essential Qualifications:</strong></p> <ul> <li style="font-weight: 400;"><span style="font-weight: 400;">Bachelor's degree </span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Ability to work independently and remain focused on task completion</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Excellent ability to interact and communicate with students of all ages, parents, and school personnel</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Strong written communication skills</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Excellent motivation and ability to complete a multi-step process involving many participants</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Excellent organizational skills</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Computer access and literacy to be able to use relevant web-based software</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Proactive; problem-solver; and self-driven&nbsp;</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Good organizational and time-management skills</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Valid NJ School Social Work Certification</span></li> <li style="font-weight: 400;">Experience in Social Work for an underserved population</li> </ul> <p>&nbsp;</p> <p></p> <h3 data-section-id="knjhsg" data-start="4331" data-end="4362"><strong data-start="4335" data-end="4362">Compensation &amp; Benefits</strong></h3> <ul data-start="4363" data-end="4594"> <li data-section-id="154svom" data-start="4363" data-end="4423">Salary range (65k - 85k) based on experience and certification</li> <li data-section-id="1syehh6" data-start="4424" data-end="4458">Comprehensive benefits package</li> <li data-section-id="lgff29" data-start="4459" data-end="4522">Instructional coaching and ongoing professional development</li> <li data-section-id="e1z5sm" data-start="4523" data-end="4594">Leadership and growth opportunities within a high-performing school</li> </ul> <p></p> <p><span style="font-weight: 400;">&nbsp;</span></p>
