You will follow individualized care plans, administer treatments, and collaborate with our interdisciplinary team to ensure optimal patient outcomes.</p> <hr data-start="1404" data-end="1407"> <h3 data-start="1409" data-end="1439"><strong data-start="1413" data-end="1437">Key Responsibilities</strong></h3> <ul data-start="1440" data-end="2190"> <li data-start="1440" data-end="1520"> <p data-start="1442" data-end="1520">Deliver direct patient care in accordance with the State Nurse Practice Act.</p> </li> <li data-start="1521" data-end="1611"> <p data-start="1523" data-end="1611">Implement nursing practices based on comprehensive patient assessments and care plans.</p> </li> <li data-start="1612" data-end="1662"> <p data-start="1614" data-end="1662">Document patient care accurately and promptly.</p> </li> <li data-start="1663" data-end="1751"> <p data-start="1665" data-end="1751">Provide patient and family education regarding diagnoses, treatments, and self-care.</p> </li> <li data-start="1752" data-end="1852"> <p data-start="1754" data-end="1852">Coordinate with hospice services, including aides, OT, PT, MSW, ST, dietitians, and supervisors.</p> </li> <li data-start="1853" data-end="1912"> <p data-start="1855" data-end="1912">Use equipment and supplies efficiently and effectively.</p> </li> <li data-start="1913" data-end="2008"> <p data-start="1915" data-end="2008">Manage pain and other symptoms, evaluating patient responses to treatments and medications.</p> </li> <li data-start="2009" data-end="2116"> <p data-start="2011" data-end="2116">Participate in personal and professional growth, quality improvement initiatives, and team development.</p> </li> <li data-start="2117" data-end="2190"> <p data-start="2119" data-end="2190">Perform other duties as assigned by the supervising registered nurse.</p> </li> </ul> <hr data-start="2192" data-end="2195"> <h3 data-start="2197" data-end="2221"><strong data-start="2201" data-end="2219">Qualifications</strong></h3> <ul data-start="2222" data-end="2754"> <li data-start="2222" data-end="2292"> <p data-start="2224" data-end="2292">Graduate of an accredited practical or vocational nursing program.</p> </li> <li data-start="2293" data-end="2404"> <p data-start="2295" data-end="2404">Minimum of 3 years nursing experience; hospice, community health, or medical/surgical experience preferred.</p> </li> <li data-start="2405" data-end="2446"> <p data-start="2407" data-end="2446">Current LPN/LVN license in the state.</p> </li> <li data-start="2447" data-end="2514"> <p data-start="2449" data-end="2514">Committed to professional standards and nursing best practices.</p> </li> <li data-start="2515" data-end="2582"> <p data-start="2517" data-end="2582">Strong communication, organizational, and interpersonal skills.</p> </li> <li data-start="2583" data-end="2641"> <p data-start="2585" data-end="2641">Valid driver’s license with insured, reliable vehicle.</p> </li> <li data-start="2642" data-end="2672"> <p data-start="2644" data-end="2672">Current CPR certification.</p> </li> <li data-start="2673" data-end="2754"> <p data-start="2675" data-end="2754">Understanding of hospice philosophy and the needs of terminally ill patients.</p> </li> </ul> <hr data-start="2756" data-end="2759"> <p data-start="2761" data-end="3033"><strong data-start="2761" data-end="2791">Why Join Cabrillo Hospice?</strong><br data-start="2791" data-end="2794">At Cabrillo, you’re not just joining a workplace, you’re joining a family that values compassion, excellence, and professional growth.
