Our interdisciplinary approach ensures seamless care coordination, strong clinical partnerships, and exceptional support across the continuum of care.</p> <p data-start="807" data-end="916">We are seeking an experienced and relationship-driven Hospice Account Executive to join our LA County/North OC team.</p> <h3 data-start="918" data-end="939">Position Overview</h3> <p data-start="941" data-end="1277">The Hospice Account Executive plays a critical role in managing and strengthening relationships with referral partners while ensuring continuity of care for patients transitioning into hospice services. This role focuses on account management, education, care coordination, and collaboration with internal clinical teams—not sales quotas.</p> <h3 data-start="1279" data-end="1303">Key Responsibilities</h3> <p data-start="1305" data-end="1342"><strong data-start="1305" data-end="1342">Relationship &amp; Account Management</strong></p> <ul data-start="1343" data-end="1666"> <li data-start="1343" data-end="1550"> <p data-start="1345" data-end="1550">Develop, manage, and maintain strong relationships with assigned referral partners, including hospitals, physicians, skilled nursing facilities, assisted living communities, and other healthcare providers.</p> </li> <li data-start="1551" data-end="1666"> <p data-start="1553" data-end="1666">Serve as the primary point of contact for referral sources, ensuring consistent communication and responsiveness.</p> </li> </ul> <p data-start="1668" data-end="1689"><strong data-start="1668" data-end="1689">Care Coordination</strong></p> <ul data-start="1690" data-end="1968"> <li data-start="1690" data-end="1853"> <p data-start="1692" data-end="1853">Collaborate closely with discharge planners, case managers, social workers, and clinical teams to ensure smooth and timely patient transitions into hospice care.</p> </li> <li data-start="1854" data-end="1968"> <p data-start="1856" data-end="1968">Monitor referral processes to ensure patient needs, expectations, and care plans are clearly understood and met.</p> </li> </ul> <p data-start="1970" data-end="1993"><strong data-start="1970" data-end="1993">Education &amp;
