font-family: 'times new roman', times, serif;"><strong>Essential Functions:</strong><br>Collaborates with hospice interdisciplinary team, hospitals, and care facilities to develop comfort-focused plans of care,&nbsp;complete comprehensive assessments, and evaluate ongoing interventions.</span></p> <p><span style="font-size: 12pt; font-family: 'times new roman', times, serif;">Collaborates with patients, families, and case managers during hospital discharge planning to ensure safe and appropriate transitions of care.</span></p> <p><span style="font-size: 12pt; font-family: 'times new roman', times, serif;">Conducts joint home visits with admissions nurses when psychosocial complexity is present (e.g., patients living alone, lacking caregivers, or experiencing mental health/substance use concerns).</span></p> <p><span style="font-size: 12pt; font-family: 'times new roman', times, serif;">Performs informational and follow up visits for patients in hospitals, facilities, and home settings.</span></p> <p><span style="font-size: 12pt; font-family: 'times new roman', times, serif;">Supports initial response for urgent referrals needing immediate response.<br></span></p> <p><span style="font-size: 12pt; font-family: 'times new roman', times, serif;"><strong>Qualifications:</strong></span></p> <p><span style="font-size: 12pt; font-family: 'times new roman', times, serif;">MSW required</span></p> <p><span style="font-size: 12pt; font-family: 'times new roman', times, serif;">LSW or LCSW is also required or must be obtained within 2 years of hire date.</span></p> <p><span style="font-size: 12pt; font-family: 'times new roman', times, serif;">Two years’ experience in the health care setting preferred.</span></p> <p>&nbsp;</p> <p><span style="font-size: 12pt;
