<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Full Time (open to 32, 36 or 40 hrs./week)</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">8am - 5pm</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Home Hospice mainly with additional responsibilities in the inpatient center/admissions</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">On Call - Based on program needs and subject to change</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"> On call hours are: Weeknights, 5pm - 8am next morning</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"> Approx. 2 nights every 4 weeks</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Weekends</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"> 1 weekend approx. every 4 -5 weeks</span></p>

<p><strong><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">JOB SUMMARY</span></strong></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Serve as a member of Choices Healthcare Medical Services and Palliative Medicine Team. Provide clinical and organizational leadership across settings in hospice and palliative care. Provide hospice and palliative care consultations and visits in home, inpatient, and hospital settings. Participate in the development and implementation of medical policies, procedures, and protocols. Comply with state and national regulations. Instruct medical students, residents, fellows, and other health professionals. Represent Choices Healthcare in interactions with physicians, hospital staff, nursing home staff, and others in the community.</span></p>
<p><strong><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">FUNCTIONS</span></strong></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Provide consultations and subsequent visits for Choices Healthcare and Palliative Medicine patients.</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Document visits and services according to agency standards for communication and billing.</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Help assure agency compliance with state and federal regulations.</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Consult with attending physicians and, when involved interdisciplinary teams, establish plans of care.</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Provide critical thinking and clinical judgment to ensure care plans are medically appropriate, individualized to meet physical, emotional, psychological, and spiritual needs and reflect the goals of care of patients and families receiving hospice and palliative care services.</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Prescribe medications according to state and federal standards, following organizational policies for prescribing of controlled substances.</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Ensure plans meet Choices Healthcare and Palliative Medicine standards and guidelines and are consistent with accepted hospice and palliative care standards.</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Participate in the provision of around-the-clock hospice and palliative medicine services, including on-call responsibilities and weekend/holiday coverage.</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Represent Choices Healthcare in interactions with physicians, hospital staff, nursing home staff, and others in the community.</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Instruct medical students, residents, fellows, and other health professionals seeking hospice and palliative care education.</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Provide medical direction regarding Choices Healthcare financial responsibility for medications, treatments, and procedures.</span></p>
<p><strong><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">QUALIFICATIONS:</span></strong></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Doctor of Medicine or Osteopathy degree from an accredited medical school required</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Unrestricted active PA license to practice medicine required. </span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Must be board certified or board eligible in primary medical specialty by the ABMS</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Team oriented with ability to provide personalized patient care as well as family support and demonstrate clinical outcomes. Must be able to assess, evaluate, plan, and implement care in complex and life limiting situations.</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Able to effectively communicate verbally and in writing, and to motivate, lead, and provide specialized medical direction, particularly in a team setting and also in physician-to-physician relationships.</span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Must have working knowledge of computer based programs and have the ability to learn and navigate the computer system and platforms. </span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Current driver's license in the state of Pennsylvania. Able to make home visits to patients. </span></p>
<p><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Member in good standing on the medical staff(s) of accredited hospitals in the service area.</span></p>
