We seek individuals who are driven to make a difference and embody our motto, “To Care is an Honor.” Join Ennoble Care today!</p></div><p><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><strong>Ennoble Care is seeking a full-time <em>Care Coordinator, Embedded</em> to join our team, providing on-site support within one or more partnered long-term care facilities in the <em>Waco and Temple, Texas areas.</em> This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support.</strong></span></p>
<p><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><strong>This Care Coordinator works directly within partnered facilities alongside providers, patients, families, and facility staff to support continuity of care, with a primary focus on Remote Patient Monitoring (RPM). Responsibilities include supporting RPM enrollment and compliance, coordinating follow-up care, addressing barriers to care, communicating patient needs, and maintaining accurate and timely documentation within the EMR. This role is very family-practice focused, highly relationship-driven, and centered around being proactive versus reactive when it comes to patient care.</strong></span></p>
<p><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><strong>Qualified candidates must possess a minimum of a Medical Assistant (MA) certification with at least one-year of clinical experience, and be compassionate, detail-oriented professionals who thrive in a fast-paced environment. Candidates should demonstrate strong organizational skills, adaptability, and clear, professional communication.</strong></span></p>

<p><span style="font-size: 10pt;"><strong>WHAT 'EMBEDDED' MEANS AT ENNOBLE CARE</strong></span></p>
<p><span style="font-size: 10pt;">This is not a remote or call-center role.</span></p>
<p><span style="font-size: 10pt;">As an Embedded Care Coordinator, you are physically on-site in one or more partnered long-term care facilities, working alongside providers and facility staff. You build consistent relationships with residents and families and become a trusted, familiar presence within the facility -- supporting care the way a traditional family practice would, but in the long-term care setting. To support a dedicated group of patients. <em>The work is very much family-practice focused, including a strong emphasis on remote patient monitoring (RPM) and proactive care coordination.</em></span></p>

<p><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><strong>KEY RESPONSIBILITIES</strong></span></p>
<p> </p>
<ul>
<li><span style="font-size: 10pt;">Legally authorized to work in the United States of America</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Active, unrestricted clinical certification or licensure, in the state of Texas, such as:</span>
<ul>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Medical Assistant (MA) / Certified Medical Assistant (CMA)</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Or comparable credential</span></li>
</ul>
</li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;"><em>Minimum 1 year</em> of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long-term care setting</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Well-developed organizational and time-management skills, paired with an optimistic approach and initiative-driven mindset</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Strong working knowledge of healthcare terminology, diagnoses, medications, and care coordination principles</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Proficiency with Microsoft 365, including Outlook, Word, and Excel</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Ability to work full-time, Monday through Friday, 8:30 a.m. to 5:00 p.m. CST, (or thereabouts, contingent on facility hours) in an on-site role, based in the<strong> </strong><em>Waco and Temple, Texas areas</em></span>
<ul>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">No weekends, <span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">evenings, nights, or on-call requirements</span></span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Daytime hours</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Seven (7) company-paid holidays annually</span></li>
</ul>
</li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Transportation <em>must-haves</em>:</span>
<ul>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Valid, unencumbered driver’s license</span></li>
