This on-site/in-office role serves as a key member of our interdisciplinary care team, supporting patients through proactive care coordination, collaboration, and a patient-centered approach.</strong></span></p>
<p data-start="278" data-end="668"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><strong>This Care Coordinator works closely alongside fellow Care Coordinators, providers, patients, families, and clinical staff to help ensure seamless continuity of care. Responsibilities include managing patient communication, coordinating provider schedules and visits, facilitating follow-up care, addressing barriers to care, and maintaining accurate and timely documentation within the EMR.</strong></span></p>
<p data-start="670" data-end="960"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><strong>Qualified candidates must possess a minimum of a Medical Assistant (MA) certification with at least one-year of clinical experience, and be compassionate, detail-oriented professionals who thrive in a fast-paced environment. Candidates should demonstrate strong organizational skills, adaptability, and clear, professional communication.</strong></span></p>

<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;" data-section-id="axa0hz" data-start="108" data-end="280"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Coordinate and manage patient care activities through Ennoble Care’s electronic medical record (EMR) system, supporting continuity of care and positive patient outcomes</span></li>
<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;" data-section-id="5og9mo" data-start="281" data-end="516"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Support day-to-day clinical workflows by assisting with referrals, laboratory orders and follow-up, medication refill requests, durable medical equipment (DME) coordination, and other care coordination tasks</span></li>
<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;" data-section-id="kdsgp4" data-start="517" data-end="696"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Maintain frequent communication with patients, providers, families, and interdisciplinary teams to address patient needs, resolve barriers to care, and ensure timely follow-up</span></li>
<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;" data-section-id="16gsfp8" data-start="697" data-end="861"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Document all patient interactions accurately and communicate changes in condition, concerns, or urgent matters promptly to the appropriate clinical team members</span></li>
