individualized treatment plans</p> </li> <li data-start="1625" data-end="1703"> <p data-start="1627" data-end="1703">Participate in <strong data-start="1642" data-end="1663">clinical staffing</strong> and discharge planning with care team</p> </li> <li data-start="1704" data-end="1792"> <p data-start="1706" data-end="1792">Coordinate care with <strong data-start="1727" data-end="1756">nurses, degree in Social Work, Counseling, or related behavioral health field<br data-start="2247" data-end="2250">✔️ Current licensure in the state of Arizona (LCSW, LMSW, LPC, LAC)<br data-start="2323" data-end="2326">✔️ Strong clinical assessment, planning, and documentation skills<br data-start="2391" data-end="2394">✔️ Knowledge of Medicaid, Medicare, and regional behavioral health systems<br data-start="2468" data-end="2471">✔️ Experience with crisis stabilization, case management, and therapeutic interventions<br data-start="2558" data-end="2561">✔️ Ability to work well in fast-paced,
