Other duties may be assigned.</span></p> <ul> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Coordinate comprehensive care and connect participants with medical, behavioral health, housing, education, employment, and community resources.</span></li> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Develop strong partnerships with community organizations and service providers to ensure timely referrals and continuity of care.</span></li> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Collaborate closely with clinicians to develop and implement individualized treatment and continuing care plans.</span></li> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Assist participants and families with accessing public benefits including Medi-Cal, CalFresh, housing resources, and other supportive services.</span></li> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Advocate for participants by coordinating services with healthcare providers, schools, probation, child welfare agencies, and community partners.</span></li> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Monitor referrals, follow participant progress, and ensure successful engagement with recommended services.</span></li> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Provide support during transitions between levels of care and after discharge to promote long-term recovery and stability.</span></li> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Maintain accurate documentation and care coordination records within the electronic health record system.</span></li> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Participate as an active member of the multidisciplinary treatment team while supporting participant-centered, trauma-informed care.</span></li> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Help educate participants on available resources, treatment options, and self-advocacy skills that promote independence and wellness.</span></li> </ul> <p><span style="font-size: 10pt; font-family: georgia, palatino, serif;"><strong>OTHER SKILLS AND ABILITIES</strong>:</span></p> <ul> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Strong knowledge of community resources and social service systems.</span></li> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Experience with electronic health records (SAGE/PCNX preferred).</span></li> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Familiarity with Drug Medi-Cal, SAPC requirements, and behavioral health systems.</span></li> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Excellent documentation and time management skills.</span></li> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Ability to prioritize multiple cases while maintaining attention to detail.</span></li> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Skilled in motivational interviewing, participant engagement, and collaborative problem solving.</span></li> <li style="font-size: 10pt; font-family: georgia, palatino, serif;"><span style="font-size: 10pt; font-family: georgia, palatino, serif;">Commitment to trauma-informed, culturally responsive, and strengths-based care.</span></li> </ul> <p><span style="font-size: 10pt; font-family: georgia, palatino, serif;"><strong>EDUCATION</strong>: Certificate or degree in <strong>Social Work, Psychology, Public Health, Human Services, or a related field</strong> preferred. Training in Motivational Interviewing (MI), Trauma-Informed Care, Harm Reduction, ASAM Continuum, and Cognitive Behavioral Therapy (CBT) is highly desirable.</span></p> <p><span style="font-size: 10pt;
