As a Hospice Aide/CNA, you are a valued member of the interdisciplinary team, providing essential personal care and comfort to patients at the end of life. Working under the supervision of a Registered Nurse, you help maintain dignity, ease, and peace for patients and their families. Guided by our values—trustworthy, capable, compassionate, proactive, and called—every visit is an opportunity to make a meaningful difference.</p> <p><strong>Essential Functions</strong></p> <p><strong>Direct Patient Care</strong></p> <ul> <li>Perform assigned personal care tasks promptly and compassionately; report any tasks you are unable to perform to the Case Manager immediately.</li> <li>Provide personal hygiene care, including bathing (bed, sponge, tub, shower), back rubs, oral care, shampooing, and linen changes.</li> <li>Assist with toileting, elimination activities, dressing, and undressing.</li> <li>Help with ambulation and exercise as instructed by the hospice nurse or therapist.</li> <li>Offer support with bedpans, urinals, and light laundry needs.</li> <li>Provide observation and care to prevent skin breakdown, especially over pressure areas.</li> <li>Assist with self-administered medications as allowed under state regulations (e.g., reminders).</li> </ul> <p><strong>Comfort, Nutrition &amp; Environment</strong></p> <ul> <li>Prepare nutritious meals and shop for groceries as assigned.</li> <li>Assist with feeding when needed.</li> <li>Maintain a clean, safe, and orderly patient living environment.</li> <li>Provide respite to the family/caregiver when appropriate.</li> <li>Demonstrate respect for the patient’s home, surroundings, and personal needs.</li> </ul> <p><strong>Monitoring &amp; Reporting</strong></p> <ul> <li>Observe and report changes in the patient’s condition, mood, or environment to the Case Manager.</li> <li>Be aware of caregiver dynamics and interpersonal issues within the home.</li> <li>Take and record vital signs (temperature, pulse, respirations, blood pressure) when ordered and when competency has been demonstrated.</li> </ul> <p><strong>Communication &amp; Documentation</strong></p> <ul> <li>Communicate with patients and families using respectful, supportive, and professional techniques.</li> <li>Maintain confidentiality at all times.</li> <li>Follow all organizational documentation requirements, care procedures, and standards of conduct.</li> </ul> <p><strong>Additional Duties</strong></p> <ul> <li>Participate in quality assessment and performance improvement activities as assigned.</li> </ul> <p><strong>Qualifications</strong></p> <ul> <li>Meets all state training requirements.</li> <li>Completion of at least 75 hours of classroom and supervised practical training (16 classroom hours prior to 16 hours of clinical training).</li> <li>At least 18 years old; able to read, follow written instructions, and document care.</li> <li>Understanding of hospice philosophy and comfort working with terminally ill patients.</li> <li>Demonstrates empathy for individuals who are ill, injured, frail, or impaired.</li> <li>Satisfactory professional references.</li> <li>Licensed driver with a reliable, insured automobile.</li> <li>Successful completion of a probationary period demonstrating competency.</li> <li>Current CPR certification.</li> <li>Certified Nursing Assistant credential when required by state regulation.</li> </ul> <p>&nbsp;</p>
