The HHA will provides personal care services to patient and performs related tasks in the patient/family home in accordance with the plan of treatment as assigned by the Nurse/Case Manager and in compliance with organization policies and procedures and applicable laws and regulations.</p> <p><span style="text-decoration: underline;"><strong>Schedule:</strong></span></p> <p>Part-time; 3 days per week; Monday - Friday; 8:30am - 5:00pm</p> <p><span style="text-decoration: underline;"><strong>Location:</strong></span></p> <p>San Francisco &amp; Marin Counties<strong><br><br><span style="font-size: 12pt;"><em>**MUST HAVE HHA LICENSE**</em></span></strong></p> <p><em><strong>By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.</strong></em><br><br><strong><u>Essential Duties &amp; Responsibilities include&nbsp;</u></strong>(but are not limited to):</p> <ul> <li>Assist patient in bathing, care of mouth, skin, and hair as needed</li> <li>Assist patient in and out of bed and with ambulation</li> <li>Turn patient in bed, position for comfort, and change bed linen</li> <li>Assist with prescribed exercises as directed</li> <li>Prepare meals and assist with eating if meals needed during assigned visit time</li> </ul> <p><strong><u>Education/Experience</u>:</strong></p> <p>&nbsp;</p> <ul> <li style="font-weight: bold;"><strong>Completion of an approved Home Health Aide training program</strong></li> <li>High school diploma or general education degree (GED); or related experience and/or training; or equivalent combination of education and experience</li> </ul> <p><strong><u>Certificates, Licenses, Registrations:</u></strong></p> <ul> <li><strong>Completion of an approved Home Health Aide (HHA) training program.
