The Licensed&nbsp;Nurse is responsible for providing direct patient care under the supervision of a registered nurse and in concert with our values: trustworthy, capable, compassionate, proactive, and called.&nbsp;Every encounter is an opportunity to make a meaningful difference, and we are committed to bringing a culture of caring to every encounter.</p> <p><strong>Essential Functions</strong></p> <p><strong>Patient Care</strong></p> <ul> <li>Provide direct patient care in accordance with the state Nurse Practice Act.</li> <li>Implements plan of care initiated by the registered nurse.</li> <li>Assesses and provides patient and family/caregiver education regarding self-care techniques and information&nbsp;<br>pertinent to diagnosis and plan of care.</li> <li>Use and prepare equipment and supplies safely and effectively, consistently adhering to aseptic technique.</li> </ul> <p><strong>Communication &amp; Coordination</strong></p> <ul> <li>Prepares clinical notes in accordance with professional licensing body and internal policy using the company’s electronic&nbsp;<br>medical record and submits documentation timely.</li> <li>Attends and contributes to in-services, case conferences, and other meetings as required by agency policy to ensure&nbsp;<br>coordinated and comprehensive plans of care for the patients of the agency.</li> <li>Interacts with team members in a positive and professional manner.</li> <li>Communicates plan of care to referring physician and other agency professionals.</li> <li>Maintains contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient&nbsp;<br>care</li> </ul> <p><strong>Additional Responsibilities</strong></p> <ul> <li>&nbsp;Participates in personal and professional growth and development.</li> <li>Performs other duties as assigned by the registered nurse.</li> </ul> <p><strong>Qualifications</strong></p> <ul> <li>&nbsp;Graduate of an accredited Licensed Practical Nurse or Licensed Vocational Nurse program.</li> <li>Three (3) years nursing experience, preferred. Community, home health or medical/surgical experience is preferred.</li> <li>Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.</li> <li>Possess and maintain valid CPR certification.</li> <li>Complies with accepted professional standards and practice.</li> <li>Demonstrates good verbal and written communication and organizational skills.</li> <li>Licensed driver with reliable transportation.</li> </ul> <p>&nbsp;</p>
