We are looking for a Care Manager for our team in Indiana. This is a field-based position that requires frequent travel to patient's homes in the Fort Wayne, IN area. Program & Position Overview: The Care Manager will be a vital part of our operations in Indiana. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $22.00 and $26.00 an hour at the commencement of your first year of employment.
