About the Role Help patients regain strength, mobility, and independence in the comfort of their own homes. As a Home Health Physical Therapist Assistant (PTA), you'll provide skilled therapy services under the direction of a Physical Therapist, implementing individualized treatment plans that help patients recover from illness, injury, or surgery while improving their ability to safely perform everyday activities. Through compassionate, hands-on care, you'll play an important role in helping patients achieve their rehabilitation goals and enhance their quality of life. This role is ideal for a dedicated PTA who enjoys building meaningful patient relationships and making a lasting impact through home-based care. You'll deliver therapeutic exercises and interventions, monitor patient progress, educate patients and caregivers on mobility, safety, and equipment use, and communicate changes in condition with the interdisciplinary care team.
