<p><strong>Schedule:</strong> Monday–Friday, 8:30 AM–5:00 PM EST (subject to business needs)<br><strong>Location:</strong> Hybrid (Syracuse, NY) — 3 days onsite / 2 days remote following successful completion of onsite training<br><strong>Bonus:</strong> $1,000 retention bonus</p>
<p>We’re looking for a <strong>Patient Referral Coordinator</strong> who wants their work to mean something — someone who brings care, accountability, and purpose to what they do every day.</p>

By coordinating referrals accurately and efficiently, this role helps remove barriers to treatment and supports a smooth, patient‑first experience.</p>

<h3>What You’ll Do</h3>
<p>In this role, you will:</p>
<ul>
<li>Ensure patient referrals are fully prepared for service by gathering required documentation, confirming insurance coverage, and securing authorizations in compliance with all regulatory requirements</li>
<li>Coordinate new infusion and enteral referrals from intake through initial processing, ensuring accuracy, timeliness, and clarity at every step</li>
<li>Collaborate closely with referral sources, hospital liaisons, pharmacy, nursing, billing, and branch teams to support seamless care delivery</li>
<li>Communicate coverage details and financial obligations clearly to patients or their representatives, ensuring understanding and agreement prior to service</li>
<li>Maintain accurate patient records, referral logs, and authorization tracking to support timely billing and reduce delays in reimbursement</li>
<li>Identify opportunities to improve intake and referral workflows while maintaining high standards of service and compliance</li>
<li>Bring reliability, follow‑through, and thoughtful problem‑solving to your daily work</li>

<p><strong>Qualifications</strong></p>
<ul>
<li>Associate’s degree in Business, Healthcare Services, or a related field required</li>
<li>Minimum of one (1) year of healthcare experience required</li>
<li>Strong knowledge of payer requirements related to billing, authorizations, and coverage</li>
<li>Proficiency with computer systems and documentation accuracy</li>
<li>Strong organizational, multitasking, and problem‑resolution skills</li>
</ul>
<p><strong>Preferred</strong></p>
<ul>
<li>Two (2) or more years of experience with a home care or infusion provider</li>
<li>Prior home infusion experience on the provider or payer side</li>
</ul>
