WCDC seeks to renew hope and purpose for those battling opioid use by removing barriers, providing a holistic approach to recovery, and utilizing evidence-based treatment to restore healthy living. Our individual service plans are built around patients in a supportive, safe environment to create a personalized&nbsp;</span><span data-contrast="auto">pathway to recovery. In addition to recovery, we also offer planned patient transportation and family and social support.</span><span data-ccp-props="{}">&nbsp;</span></p> <p><strong><span style="font-size: 10pt;">Position Summary:</span></strong></p> <p><span style="font-size: 10pt;">The Care Navigator plays a pivotal role in supporting patient care and engagement by providing on-demand peer services, crisis intervention, and de-escalation support. Stationed in the lobby, the Care Navigator serves as a vital resource for patient engagement, pre-registration assistance, and overall care coordination. Acting as a bridge between patients and healthcare providers, the Care Navigator ensures seamless access to services and support throughout the patient's care journey.</span></p> <h3><span style="font-size: 10pt;">Key Responsibilities:</span></h3> <h4><span style="font-size: 10pt;">Patient Engagement</span></h4> <ul> <li><span style="font-size: 10pt;">Assist patients with the Web-App and other pre-registration tasks, ensuring they receive the support needed to navigate their healthcare journey.</span></li> </ul> <h4><span style="font-size: 10pt;">Crisis Intervention</span></h4> <ul> <li><span style="font-size: 10pt;">Provide immediate support for mental health crises, including suicidal or homicidal ideation, by employing motivational interviewing and de-escalation techniques.</span></li> </ul> <h4><span style="font-size: 10pt;">Queue Management</span></h4> <ul> <li><span style="font-size: 10pt;">Monitor patient queues to ensure timely service, alerting the SUD team if a patient has been waiting longer than 5 minutes.</span></li> </ul> <h4><span style="font-size: 10pt;">Crisis Support</span></h4> <ul> <li><span style="font-size: 10pt;">Offer emotional and de-escalation support across departments, helping patients navigate overwhelming feelings and barriers to care.</span></li> </ul> <h4><span style="font-size: 10pt;">Administrative Support</span></h4> <ul> <li><span style="font-size: 10pt;">Manage critical reports, including the 45/90-day report and UA intervention report.</span></li> <li><span style="font-size: 10pt;">Support SUD teams with rooming and patient care as needed.</span></li> </ul> <h4><span style="font-size: 10pt;">Departmental Coverage</span></h4> <ul> <li><span style="font-size: 10pt;">Provide backup support for the PSR desk and the Neighborhood Center.</span></li> <li><span style="font-size: 10pt;">Answer questions related to the SUD/Peer program and assist with overall clinic operations.</span></li> </ul> <h3><span style="font-size: 10pt;">Additional Duties:</span></h3> <ul> <li><span style="font-size: 10pt;">Arrange patient transportation as needed.</span></li> <li><span style="font-size: 10pt;">Generate billable check-ins for peer services.</span></li> <li><span style="font-size: 10pt;">Advocate for patients, helping them overcome barriers to care.</span></li> <li><span style="font-size: 10pt;">Ensure smooth transitions for unenrolled and outreach patients.</span></li> </ul> <h3><span style="font-size: 10pt;">Qualifications:</span></h3> <ul> <li><span style="font-size: 10pt;">Strong interpersonal and communication skills.</span></li> <li><span style="font-size: 10pt;">Experience in crisis intervention and motivational interviewing preferred.</span></li> <li><span style="font-size: 10pt;">Familiarity with healthcare or peer support services is a plus.</span></li> </ul> <p><span style="font-size: 10pt;">The Care Navigator is instrumental in fostering a supportive and accessible healthcare environment, ensuring every patient feels empowered and cared for throughout their journey.</span></p> <h3><span style="font-size: 10pt;">Equal Employment Opportunity (EEO) Statement</span></h3> <p><span style="font-size: 10pt;">We Care Daily Clinics is an Equal Opportunity Employer.
