The ultimate goal of our programming is to provide each individual with coaching, education, tools and access to community resources that will help them successfully maintain long-term sustained housing, financial independence and emotional well-being.</p> </td> </tr> <tr> <td> <p><strong>Responsibilities:</strong></p> <ul> <li>Empower residents with exercising autonomy and independence in all aspects of their life and assists with identifying choices and developing skills in self-advocacy.</li> <li>Conduct initial needs assessment and collaboratively develop a plan with the resident to obtain benefits/entitlements and other community-based resources.</li> <li>Collaborate across the staff team to ensure that resident perspective is always incorporated, that preferences are recognized and that choices are maximized.</li> <li>Support new residents in all phases of transitioning into housing in collaboration with partnering organizations.&nbsp;&nbsp;&nbsp;</li> <li>Assist clients in being fully integrated into their communities by supporting them with resource utilization and/or obtaining memberships and natural supports.&nbsp;&nbsp;</li> <li>Provide practical help and mentoring/modeling, advocacy, coordination, side-by-side individualized support, problem solving and direct assistance to help clients obtain skills needed for daily independent living.</li> <li>Promote concepts of health and wellness by assisting clients with accessing information and services related to medical care including HIV care resources, nutrition, and exercise as appropriate.</li> <li>Intervene in potential crisis situations, utilizing trauma informed, verbal de-escalation techniques.&nbsp;</li> </ul> </td> </tr> <tr> <td> <p><strong>Qualifications:</strong></p> <ul> <li>Bachelor’s degree or Master’s degree in a human services field or social work or related field, preferred</li> <li>For candidates with lived experience with substance use disorder who have 2 years sustained sobriety: High school diploma/GED and at least one year of paid or volunteer experience working with adults with mental illness and/or substance abuse.&nbsp;&nbsp;</li> </ul> <p><strong>Compensation:</strong></p> <ul> <li>Bachelors: $48,000.00 annually</li> <li>Masters: $50,000.00 annually</li> </ul> </td> </tr> <tr> <td> <p><strong>Schedule: </strong>Monday-Friday, 9am-5pm</p> </td> </tr> </tbody> </table><div class="content-pay-transparency"><div class="pay-input"><div class="title">Annual Salary</div><div class="pay-range"><span>$48,000</span><span class="divider">&mdash;</span><span>$50,000 USD</span></div></div></div><div class="content-conclusion"><p>At Eliot, we are committed to supporting you at every stage of your career, providing opportunities for growth, purpose, and community.
