As a Physician Services Clinical Coordinator, you'll serve as the central point of connection between providers, patients, caregivers, and healthcare partners, helping navigate complex healthcare needs and ensuring care plans are carried out smoothly and efficiently. Through thoughtful coordination, communication, and follow-through, you'll help create a seamless care experience for elderly and medically complex patients.
