Eliot's Health Home program is an innovative care management approach for adults with complex medical, behavioral health, and social needs. The ideal candidate for this position will have a strong commitment to mental health recovery, knowledge of substance abuse disorders and commitment to coordination and integration of all aspects of health care. Additionally, the candidate should possess strong time-management, organizational and communication skills.</p> </td> </tr> <tr> <td> <p><strong>Responsibilities:</strong></p> <ul> <li>Coordinate care with the member and their medical, health, substance use, mental health and other related providers to ensure integrated treatment and care</li> <li>Provide and coordinate post hospital discharge follow-up, as needed</li> <li>Maintain contact with payer systems communicating and documenting coordination activities and service authorizations</li> <li>Provide initial and on-going member engagement, support and care coordination.&nbsp;</li> <li>Partner with the member to develop and implement a person-centered care plan.</li> <li>Connect members with community services and resources&nbsp;</li> <li>Convene interdisciplinary care team and coordinate activities to ensure integration&nbsp;&nbsp;</li> <li>Promote hope and empowerment through meetings and support with members&nbsp;</li> <li>Facilitate access to transportation/ mobility resources that enable members to fully participate in medical services, behavioral health services and community resources</li> <li>Ensure that clients are treated with dignity and respect in accordance with ECHS’ policy</li> </ul> </td> </tr> <tr> <td> <p><strong>Qualifications:</strong></p> <ul> <li>Requires a Bachelors degree in Psychology, Social Work or related field</li> <li>Experience working in care management or case management for adults</li> <li>Valid Massachusetts drivers license&nbsp;</li> </ul> </td> </tr> <tr> <td> <p><strong>Schedule: </strong>Monday-Friday, 9am-5pm</p> </td> </tr> </tbody> </table><div class="content-pay-transparency"><div class="pay-input"><div class="title">Annual Salary</div><div class="pay-range"><span>$50,000</span><span class="divider">&mdash;</span><span>$50,000 USD</span></div></div></div><div class="content-conclusion"><p>At Eliot, we are committed to supporting you at every stage of your career, providing opportunities for growth, purpose, and community.
