This role is ideal for a mission-driven clinician who values a patient-centered, root-cause approach to care and is comfortable stepping into an established practice to provide high-quality coverage.  ASSIGNMENT DETAILS:  <ul> <li>Flexible locum assignment (full-time or part-time options available) </li> <li>Coverage needed across the following clinic locations: (Medford, Central Point, Talent, Grants Pass, Coos Bay, Roseburg, North Bend, White City, Ashland, Klamath Falls) </li> <li>Assignment length: 3 months with option to extend </li> <li>This is a non-benefited position </li> </ul> RESPONSIBILITIES AND DUTIES:   <ul> <li>Provide direct patient care through office visits, telehealth, and occasional same-day appointments  </li> <li>Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions  </li> <li>Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions  </li> <li>Educate and empower patients on preventive health and functional medicine principles  </li> <li>Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff  </li> <li>Document visits in the EHR (NextGen) in a timely and compliant manner  </li> <li>Participate in team meetings, quality improvement initiatives, and training sessions  </li> <li>Support clinic operations by contributing to a positive, team-oriented environment  </li> </ul> REQUIRED QUALIFICATIONS   <ul> <li>Active Oregon license as a Nurse Practitioner*, PA-C, MD, or DO (or eligibility to obtain)  </li> <li>Board certification in Family Medicine, Internal Medicine, or applicable specialty  </li> <li>Minimum 2 years of clinical experience preferred (functional/integrative medicine experience is a plus)  </li> <li>Strong interest in preventive, functional, and lifestyle medicine  </li> <li>Excellent communication and patient relationship skills  </li> <li>Comfortable working in a fast-paced, collaborative environment  </li> <li>Proficient in using electronic health records </li> </ul> *Nurse Practitioner Licenses Accepted:    <ul> <li>FNP, Family Nurse Practitioner  </li> <li>AGNP- PC, Adult Gerontological Nurse Practitioner (Primary Care)</li> </ul>  <div class="content-conclusion">Benefits: <ul> <li style="font-size: 12pt;">Medical, Dental, Vision, and Life Insurance</li> <li style="font-size: 12pt;">401(k) Retirement Plan with Company Match</li> <li style="font-size: 12pt;">Voluntary LTD, FSA, Accident, Critical Illness</li> <li style="font-size: 12pt;">Paid Time Off and Paid Holidays</li> <li style="font-size: 12pt;">Employee and Family Discounts</li> <li style="font-size: 12pt;">Paid Parental Leave</li> </ul></div>
