About the Role

Hospice BAYADA at Inspira is looking for an RN Transitional Care Manager, who will build and strengthen relationships between referral sources and BAYADA at Inspira. This individual will focus on relationships with hospital and facility contacts to educate their leaders about BAYADA services and the benefits of hospice care. Use clinical knowledge to educate patients and hospital staff about hospice eligibility and expectations. Manage referral intake and coordinate a safe discharge to next care setting or initiating GIP level of Care.   MINIMUM QUALIFICATIONS:  <ol> <li data-aria-posinset="1" data-aria-level="2">Exemplifies characteristics of The BAYADA Way: compassion, excellence and reliability. </li> <li data-aria-posinset="2" data-aria-level="2">Current license in good standing as a Registered Nurse in the state of practice. </li> <li data-aria-posinset="3" data-aria-level="2">Graduate of an accredited and approved program as indicated by school transcript or diploma. </li> <li data-aria-posinset="4" data-aria-level="2">Two (2) year experience in health care, preferably in one these settings: Hospice RN, Clinical Liaison, RN Discharge Planner, RN Educator. </li> <li data-aria-posinset="5" data-aria-level="2">Completed Continuing Education Requirements (CEUs) as required by regulation. </li> <li data-aria-posinset="6" data-aria-level="2">Demonstrated record of building and maintaining referral relationships. </li> <li data-aria-posinset="7" data-aria-level="2">Demonstrated knowledge of the Medicare COPs and other program requirements, as applicable. </li> <li data-aria-posinset="8" data-aria-level="2">Previous discharge planning experience preferred. </li> <li data-aria-posinset="9" data-aria-level="2">Ability to provide excellent customer service. </li> <li data-aria-posinset="10" data-aria-level="2">Demonstrated ability to work well in a team environment and matrix organization </li> <li data-aria-posinset="11" data-aria-level="2">Working knowledge of business office technologies: PC/laptop w Microsoft Office applications, smartphone, website/internet, email </li> </ol>    PRIMARY RESPONSIBILITIES:  <ol> <li data-aria-posinset="1" data-aria-level="2">Demonstrate and communicate the core values of BAYADA and The BAYADA Way. </li> <li data-aria-posinset="2" data-aria-level="2">Develop relationships, in accordance with The BAYADA Way, with referral sources, physicians and other healthcare professionals. </li> <li data-aria-posinset="3" data-aria-level="2">Sell BAYADA services by clearly articulating the features and benefits of BAYADA to generate growth in the volume of referrals generated from assigned accounts. </li> <li data-aria-posinset="4" data-aria-level="2">Demonstrate mastery of clinical and regulatory hospice care eligibility criteria. </li> <li data-aria-posinset="5" data-aria-level="2">Review and compile all documentation necessary, in coordination with referral sources, to transition care from an acute care setting/ sub-acute care setting or initiate care as ordered by a physician to BAYADA Hospice. </li> <li data-aria-posinset="6" data-aria-level="2">Coordinate with the service office to set up the services ordered by the physician. </li> <li data-aria-posinset="7" data-aria-level="2">Completes the evaluation on hospital referred patients for community and for inpatient hospice admissions. </li> <li data-aria-posinset="8" data-aria-level="2">. Provides accurate and timely information regarding hospice team and hospice staff. </li> <li data-aria-posinset="9" data-aria-level="2">Participate in office’s quarterly business planning and market analysis to achieve growth goals </li> <li data-aria-posinset="10" data-aria-level="2">. Serves as patient advocate in evaluating/developing plan of care for GIP patients and obtaining informed Consent for hospice. </li> <li data-aria-posinset="11" data-aria-level="2">Providing daily GIP visits and documenting to GIP eligibility. </li> <li data-aria-posinset="12" data-aria-level="2">Collaboration with Business Development Manager and/or Director on key and shared accounts. </li> <li data-aria-posinset="13" data-aria-level="2">Attend and participate in weekly team growth meetings. </li> <li data-aria-posinset="14" data-aria-level="2">Cross-train office field staff to cover absences, as needed. </li> <li data-aria-posinset="15" data-aria-level="2">Participate in community and sales activities, as needed. </li> </ol>    BAYADA Offers:    <ul> <li>Base Salary: $90,000-94,000 / year depending on qualifications</li> <li>Performance based monthly Bonus Opportunity</li> <li>BAYADA offers a comprehensive benefits plan that includes the following: Paid holidays, vacation and sick leave, vision, dental and medical health plans, employer paid life insurance, 401k with company match, direct deposit and employee assistance program</li> <li>To learn more about BAYADA Benefits, <a href="https://www.bayada.com/benefits/">click here</a> </li> </ul>  #LIRX <div class="content-conclusion">As an accredited, regulated, certified, and licensed home health care provider, BAYADA complies with all state/local mandates.
