To learn more, visit www.habitathealth.com.</div>Role Scope: The Functional Wellness Supervisor leads and directly oversees the rehab team which includes Physical Therapists (PT), Occupational Therapists (OT), Aides, and Home Care Coordinators at the PACE center. This role ensures that therapy interventions are high-quality, participant-centered care that supports independence, safety, and quality of life. The Functional Wellness Supervisor facilitates functional wellness programming, performance improvement, and cross-departmental collaboration. This role is responsible for the day-to-day team supervision, training and competency oversight, and regulatory compliance for the rehab team working in close partnership with Center leadership and
