This role combines clinical assessment, care coordination, patient and caregiver education, and quality assurance to ensure safe, effective, and person-centered care.</p> <p style="margin-left: .375in; margin-top: 0pt; margin-bottom: 14pt; font-family: Calibri; font-size: 11.0pt;">The Care Manager, RN collaborates closely with patients, caregivers, and interdisciplinary care teams to assess needs, identify potential changes to the individualized plans of care, and facilitate the seamless initiation and delivery of services. Through regularly scheduled home visits, the RN evaluates patient and caregiver well-being, monitors the quality and effectiveness of care, and identifies opportunities to enhance health outcomes, safety, and regulatory compliance. The Care Manager, RN proactively communicates clinical concerns, changes in condition, and potential barriers to care with the appropriate team members to ensure timely intervention, effective care coordination, and optimal patient outcomes.</p> <p style="margin: 0in; This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $80,000 and $90,000 per year at the commencement of your first year of employment.
