Eliot's Program for Assertive Community Treatment (PACT) is an evidence based model providing treatment, rehabilitation and support for adults with mental illness.&nbsp; This multidisciplinary team provides all inclusive behavioral health treatment and support in the community.&nbsp;</p> <p>The ideal candidate will be dedicated to improving the lives of adults with mental health challenges and possess the interpersonal skills needed to build meaningful relationships with both the individuals we serve and members of the PACT team. Services are predominantly community based, including at consumers' homes, medical facilities, &amp; other locations.&nbsp;</p> </td> </tr> <tr> <td> <p><strong>Responsibilities:</strong></p> <ul> <li>Deliver case management services as part of a multidisciplinary team, acting as primary case manager in alignment with Eliot policies and PACT standards</li> <li>Collaborate with clients to develop individualized treatment plans and provide ongoing clinical interventions</li> <li>Conduct timely assessments to evaluate client functioning, strengths, and needs as assigned by clinical leadership</li> <li>Build therapeutic relationships to support client engagement, recovery, and goal achievement</li> <li>Participate in rotating on-call coverage and evening shifts, offering crisis support and problem-solving interventions as needed</li> </ul> </td> </tr> <tr> <td> <p><strong>Qualifications:</strong></p> <ul> <li>Requires a Bachelor’s degree in Psychology or related field</li> <li>Experience working with adults with severe mental illness</li> <li>Valid Massachusetts drivers license&nbsp;</li> </ul> </td> </tr> <tr> <td> <p><strong>Schedule: </strong>Monday - Friday 8am to 4pm with rotating evening shifts&nbsp;</p> <p>&nbsp;</p> <p>Bilingual candidates fluent in a needed language may be eligible for a 6% salary increase</p> </td> </tr> </tbody> </table> <p>&nbsp;</p><div class="content-pay-transparency"><div class="pay-input"><div class="title">Annual Salary</div><div class="pay-range"><span>$48,000</span><span class="divider">&mdash;</span><span>$48,000 USD</span></div></div></div><div class="content-conclusion"><p>At Eliot, we are committed to supporting you at every stage of your career, providing opportunities for growth, purpose, and community.
