<p>The&nbsp;Community&nbsp;Liaison,&nbsp;Home&nbsp;Health&nbsp;is&nbsp;responsible&nbsp;for&nbsp;inbound/outbound&nbsp;communication&nbsp;and&nbsp;business&nbsp;development&nbsp;across&nbsp;all&nbsp;eligible&nbsp;business&nbsp;units.&nbsp;The&nbsp;role&nbsp;acts&nbsp;as&nbsp;the&nbsp;local&nbsp;market&nbsp;B2B,&nbsp;B2C,&nbsp;and&nbsp;DTC&nbsp;leader,&nbsp;externally&nbsp;facing&nbsp;our&nbsp;community&nbsp;of&nbsp;referral&nbsp;partnerships.&nbsp;The&nbsp;Community&nbsp;Liaison,&nbsp;Home&nbsp;Health&nbsp;will&nbsp;explain&nbsp;benefits&nbsp;of&nbsp;enrollment&nbsp;and&nbsp;will&nbsp;provide&nbsp;patients,&nbsp;caregivers,&nbsp;POAs,&nbsp;and&nbsp;guardians&nbsp;with&nbsp;the&nbsp;myriads&nbsp;of&nbsp;services&nbsp;we&nbsp;bring&nbsp;to&nbsp;eligible&nbsp;patients&nbsp;in&nbsp;their&nbsp;respective&nbsp;market.</p> <li>Market&nbsp;analysis,&nbsp;along&nbsp;with&nbsp;competitive&nbsp;landscape,&nbsp;in&nbsp;which&nbsp;to&nbsp;differentiate&nbsp;our&nbsp;services&nbsp;to&nbsp;external&nbsp;referral&nbsp;sources</li> <li>Meet&nbsp;and&nbsp;exceed&nbsp;agreed&nbsp;upon&nbsp;F2F&nbsp;and&nbsp;virtual&nbsp;meeting&nbsp;goals&nbsp;with&nbsp;key&nbsp;decision&nbsp;makers</li> <li>Thoroughly&nbsp;understand&nbsp;eligibility&nbsp;and&nbsp;enrollment&nbsp;process&nbsp;of&nbsp;the&nbsp;Alpine&nbsp;Care&nbsp;suite&nbsp;of&nbsp;businesses&nbsp;in&nbsp;a&nbsp;given&nbsp;market</li> <li>Track&nbsp;performance&nbsp;in&nbsp;CRM&nbsp;demonstrating&nbsp;plan&nbsp;and&nbsp;adherence&nbsp;to&nbsp;budgeted&nbsp;targets&nbsp;for&nbsp;a&nbsp;given&nbsp;region</li> <li>Schedule&nbsp;face&nbsp;to&nbsp;face&nbsp;or&nbsp;phone&nbsp;discussions&nbsp;with&nbsp;patients,&nbsp;POAs,&nbsp;or&nbsp;guardians&nbsp;to&nbsp;provide&nbsp;guidance&nbsp;on&nbsp;patient&nbsp;eligibility&nbsp;for&nbsp;all&nbsp;programs</li> <li>Provide&nbsp;direction&nbsp;on&nbsp;benefit&nbsp;enrollment&nbsp;program&nbsp;and&nbsp;processes</li> <li>Obtain&nbsp;Voluntary&nbsp;Alignment&nbsp;Form&nbsp;from&nbsp;patients,&nbsp;POAs,&nbsp;or&nbsp;guardians&nbsp;at&nbsp;all&nbsp;times</li> <li>Answer&nbsp;inbound&nbsp;calls&nbsp;from&nbsp;patients,&nbsp;families,&nbsp;and&nbsp;facility&nbsp;staff&nbsp;members&nbsp;regarding&nbsp;various&nbsp;eligible&nbsp;programs</li> <li>Promptly,&nbsp;accurately,&nbsp;professionally&nbsp;triage&nbsp;all&nbsp;patient&nbsp;concerns&nbsp;related&nbsp;to&nbsp;care&nbsp;needs,&nbsp;scheduling&nbsp;and&nbsp;billing&nbsp;to&nbsp;the&nbsp;appropriate&nbsp;department</li> <li>Oversee&nbsp;and&nbsp;document&nbsp;outreach&nbsp;calls&nbsp;conducted,&nbsp;and&nbsp;meetings&nbsp;scheduled&nbsp;with&nbsp;external&nbsp;sources</li> <li>Ensure&nbsp;scheduled&nbsp;meetings&nbsp;with&nbsp;patients&nbsp;and&nbsp;their&nbsp;caregivers&nbsp;are&nbsp;attended&nbsp;timely</li> <li>Adhere&nbsp;to&nbsp;company&nbsp;policies&nbsp;and&nbsp;procedures&nbsp;related&nbsp;to&nbsp;patient&nbsp;outreach&nbsp;calls</li> <li>Complies&nbsp;with&nbsp;PHI&nbsp;and&nbsp;HIPPA&nbsp;guidelines</li> <li>Maintains&nbsp;consistent&nbsp;communication&nbsp;with&nbsp;Director-&nbsp;Enterprise&nbsp;Sales,&nbsp;Practice&nbsp;Managers,&nbsp;Home&nbsp;Health&nbsp;Administrators,&nbsp;and&nbsp;local&nbsp;BU&nbsp;support&nbsp;staff</li> <li>Performs&nbsp;other&nbsp;additional&nbsp;duties&nbsp;as&nbsp;assigned</li> </ul> <p>&nbsp;</p> <p>&nbsp;</p> <p><strong>Required Knowledge, Skills and Experience</strong></p> <ul> <li>High school diploma or GED&nbsp;</li> <li>At least 1 year of experience in a clinical setting or outside sales</li> <li>Must maintain a valid driver’s license and good driving record</li> <li>Excellent written and verbal communication skills</li> <li>Ability to work independently and complete multiple tasks</li> <li>Strong organizational skills</li> <li>Computer skills including but not limited to Word, <p><strong>Preferred Knowledge, Skills and Experience</strong></p> <ul> <li>Hospital experience strongly preferred</li> <li>Social workers are encouraged to apply.</li> <li>Must be a natural
