PACT is an evidence-based model that provides comprehensive treatment, rehabilitation, and support to adults with severe and persistent mental illness.&nbsp; This PACT team is designed to work with a specialized population with a history of criminal engagement.&nbsp;</p> <p>The ideal candidate will have an understanding of the unique needs and history/background of the population, clinical interventions and have the ability to develop and maintain clear and therapeutic boundaries. A proven experience in community-based work with adults with severe and persistent mental illness is a must.</p> </td> </tr> <tr> <td> <p><strong>Responsibilities:</strong></p> <ul> <li>Provide outreach support services to forensic PACT clients and medication supports as needed to clients living independently in the community.</li> <li>Provide side by side interventions to consumers that focus on problem solving, skills training, modeling behavior, optimism and encouragement.
