care.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Complete and oversee client assessments, care planning, service updates, and clinical documentation as appropriate.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Monitor client needs and collaborate with clients, families, caregivers, providers, and office leadership to promote continuity of care.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Provide coaching, education, and ongoing support to caregivers and field staff.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Assist with orientation, competency validation, performance feedback, and class="wnfdntn wnfdntm _1ibi0s35e _1ibi0s36a">Qualifications</h2> <ul class="wnfdntj"> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Current, unrestricted Registered Nurse license in the applicable state.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Clinical leadership, home care, community-based care, long-term care, or related experience preferred.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Strong assessment, care planning, documentation, and communication skills.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Ability to coach and support caregivers and work collaboratively with an interdisciplinary team.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Sound clinical judgment, strong organization, and the ability to prioritize in a
