Our team believes in every client’s ability to achieve recovery and rebuild meaningful, engaged lives in their communities.</div></div><h2><strong>Position: Case Manager (CDCA)</strong></h2> <p><strong>Job Summary:</strong> Provide support services and foster community by assessing the needs of their clients and developing and implementing a comprehensive plan to address those needs. This position reports to a Case Management Supervisor.&nbsp;&nbsp;</p> <p><strong>Duties&nbsp;and&nbsp;Responsibilities:&nbsp;</strong></p> <ul> <li>Coordinates&nbsp;and&nbsp;provides&nbsp;safe,&nbsp;timely,&nbsp;effective,&nbsp;efficient,&nbsp;equitable,&nbsp;culturally&nbsp;sensitive,&nbsp;and&nbsp;client- centered care.&nbsp;</li> <li>For&nbsp;SUD&nbsp;Clients:&nbsp;Handles&nbsp;case&nbsp;assignments,&nbsp;assesses&nbsp;barriers&nbsp;to&nbsp;recovery&nbsp;in&nbsp;the&nbsp;various&nbsp;life&nbsp;domains, may draft SUD case&nbsp;management-specific service plan, review progress in case management goals.&nbsp;</li> <li>For&nbsp;MH&nbsp;Clients:&nbsp;Handles&nbsp;case&nbsp;assignments,&nbsp;assesses&nbsp;barriers&nbsp;to&nbsp;recovery&nbsp;in&nbsp;the&nbsp;various&nbsp;life&nbsp;domains, coordinates&nbsp;mental&nbsp;health&nbsp;treatment&nbsp;planning&nbsp;including&nbsp;goals&nbsp;for&nbsp;CPST&nbsp;services,&nbsp;provides&nbsp;input,&nbsp;and works collaboratively with the clinical counselor, psychiatrist, and/or nurse practitioner in the development of the treatment plan.&nbsp;</li> <li>Is&nbsp;familiar&nbsp;with&nbsp;the&nbsp;diagnostic&nbsp;assessment&nbsp;completed&nbsp;for&nbsp;the&nbsp;clients&nbsp;served,&nbsp;which&nbsp;includes&nbsp;a&nbsp;review&nbsp;of diagnosis, identification of current symptoms, risk, functioning,&nbsp;identified&nbsp;needs in the client's life domains shown as Z-Codes to&nbsp;determine&nbsp;eligibility for Case Management or CPST services and the level of care needed.&nbsp;</li> <li>Works&nbsp;collaboratively&nbsp;following&nbsp;up&nbsp;with&nbsp;observations&nbsp;with&nbsp;the&nbsp;clinical&nbsp;counselor,&nbsp;psychiatrist,&nbsp;and/or nurse practitioner in the development of the treatment plan.&nbsp;</li> <li>Adheres&nbsp;to&nbsp;Lighthouse's&nbsp;protocol&nbsp;for&nbsp;appropriate&nbsp;"care&nbsp;management"&nbsp;activities&nbsp;as&nbsp;part&nbsp;of&nbsp;the&nbsp;continuum of&nbsp;services to meet each client's defined needs and as defined in the treatment plan.&nbsp;</li> <li>Provides follow-up and&nbsp;appropriate referrals&nbsp;to community resources are essential. Collaboration and coordination&nbsp;of&nbsp;services&nbsp;to&nbsp;ensure&nbsp;continuity&nbsp;of&nbsp;care&nbsp;and&nbsp;efficiency&nbsp;of&nbsp;services&nbsp;across&nbsp;systems&nbsp;is&nbsp;vital&nbsp;to early recovery.&nbsp;</li> <li>Provides&nbsp;linkage&nbsp;and&nbsp;assistance&nbsp;in&nbsp;accessing&nbsp;inpatient&nbsp;or&nbsp;more&nbsp;restrictive&nbsp;treatment,&nbsp;as&nbsp;medically&nbsp;necessary.&nbsp;</li> <li>Stays&nbsp;abreast&nbsp;of&nbsp;best&nbsp;practices&nbsp;in&nbsp;modalities&nbsp;of&nbsp;intervention&nbsp;to&nbsp;maintain&nbsp;high-quality&nbsp;care&nbsp;management that is medically necessary to client specific treatment plan.&nbsp;</li> <li>Evaluates&nbsp;daily&nbsp;living&nbsp;functioning&nbsp;and&nbsp;assist&nbsp;clients&nbsp;in&nbsp;creating&nbsp;goals&nbsp;to&nbsp;improve&nbsp;daily&nbsp;living.&nbsp;</li> <li>Completes&nbsp;billing&nbsp;and&nbsp;documentation&nbsp;accurately&nbsp;in&nbsp;a&nbsp;timely&nbsp;manner.&nbsp;</li> <li>Sees&nbsp;clients&nbsp;weekly&nbsp;or&nbsp;as&nbsp;clinically&nbsp;appropriate according&nbsp;to the&nbsp;client’s&nbsp;level&nbsp;of&nbsp;care.&nbsp;</li> <li>Ensures&nbsp;all&nbsp;community&nbsp;activities&nbsp;comply&nbsp;with&nbsp;HIPAA,&nbsp;42&nbsp;CFR&nbsp;Part&nbsp;2,&nbsp;and&nbsp;Mental&nbsp;Health&nbsp;laws&nbsp;and regulations on&nbsp;confidentiality of PHI.&nbsp;</li> <li>Keeps&nbsp;abreast&nbsp;of&nbsp;community&nbsp;resources&nbsp;and&nbsp;makes&nbsp;needed&nbsp;referrals&nbsp;to&nbsp;meet&nbsp;client&nbsp;needs&nbsp;on&nbsp;a&nbsp;timely&nbsp;basis.&nbsp;</li> <li>Coordinates&nbsp;and&nbsp;collaborates&nbsp;with&nbsp;other&nbsp;community&nbsp;agencies&nbsp;regarding&nbsp;client's&nbsp;treatment&nbsp;to&nbsp;promote positive treatment&nbsp;outcomes.
