data-contrast="auto">Utilize available resources to identify patients at high risk for rehospitalization and mortality and provide an extra layer of support. </span><span data-contrast="auto">Provide personalized advice based on the patient’s unique goals and care needs. </span><span data-ccp-props="{}"> </span></li> <li><span data-contrast="none">Provide guidance to
