<h2 data-start="162" data-end="205">Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC)</h2> <p data-start="206" data-end="228"><strong data-start="206" data-end="228">Remote | Full-Time</strong></p> <p data-start="230" data-end="396">Peregrine Health partners with community health organizations to expand access to high-quality behavioral health care, delivered by supported, full-time clinicians.</p> <h3 data-start="403" data-end="429">What You'll Do</h3> <p><strong>Direct Clinical Services</strong></p> <ul> <li>Conduct comprehensive biopsychosocial assessments to evaluate client needs, strengths, and risk factors</li> <li>Provide individual, family, and group therapy using evidence-based modalities (CBT, DBT, trauma-informed care, motivational interviewing, etc.)</li> <li>Develop and implement individualized treatment plans in collaboration with clients and care team members</li> <li>Monitor and document client progress, adjusting treatment plans as clinically indicated</li> <li>Provide crisis intervention, safety planning, and stabilization services as needed</li> </ul> <p><strong>Care Coordination &amp; Advocacy</strong></p> <ul> <li>Collaborate with physicians, nurses, case managers, and community health workers to ensure integrated, whole-person care</li> <li>Connect clients with community resources including housing assistance, food security programs, transportation, legal aid, and benefits enrollment</li> <li>Facilitate warm handoffs and referrals to specialty mental health, substance use, or medical services</li> </ul> <p><strong>Documentation &amp; Compliance</strong></p> <ul> <li>Complete timely and accurate clinical documentation in the electronic health record (EHR) in accordance with organizational and regulatory standards</li> <li>Maintain confidentiality and adhere to HIPAA and applicable state regulations</li> <li>Participate in quality improvement initiatives, case reviews, and outcome measurement efforts</li> </ul> <p><strong>Community Engagement</strong></p> <ul> <li>Build trusting relationships with clients, families, and community partners across diverse cultural backgrounds</li> <li>Participate in educational workshops</li> <li>Contribute to a welcoming, culturally humble, and trauma-informed organizational environment</li> </ul> <h3 data-start="403" data-end="429">What We’re Looking For</h3> <ul> <li data-start="433" data-end="492">Active Licensed Clinical Social Worker (LCSW) license or Licensed Professional Counselor (LPC/LMHC)</li> <li data-start="495" data-end="606">3+ years of post-licensure clinical experience (community mental health, integrated care, or similar settings preferred)</li> <li data-start="609" data-end="680">Comfort providing therapy independently within a supported care model</li> <li data-start="683" data-end="758">Strong clinical judgment in assessment, diagnosis, and treatment planning</li> <li data-start="761" data-end="830">Clear, professional communication and collaboration with care teams</li> <li data-start="833" data-end="902">Comfort using telehealth platforms and clinical documentation tools</li> <li data-start="905" data-end="958">Commitment to full-time work and continuity of care</li> </ul> <h3 data-start="965" data-end="992">Compensation &amp; Benefits</h3> <ul> <li data-start="996" data-end="1020">Competitive salary</li> <li data-start="1023" data-end="1070">Full medical, dental, and vision coverage</li> <li data-start="1073" data-end="1108">20 PTO days + 9 paid holidays</li> <li data-start="1111" data-end="1143">401(k) with employer match</li> <li data-start="1146" data-end="1172">Malpractice coverage</li> <li data-start="1175" data-end="1209">Continuing education support</li> <li data-start="1212" data-end="1259">$1,800 per year toward mental health care</li> <li data-start="1262" data-end="1297">$5,000 fertility care stipend</li> <li data-start="1300" data-end="1359">Full administrative, licensing, and credentialing support</li> </ul>
