<p>Our Mobile Primary Care Team is seeking a dedicated and compassionate <strong>Physician Assistant</strong> or <strong>Nurse Practitioner</strong> with a heart for senior care and a passion for redefining how older adults and their caregivers experience health and well-being. <p><strong>Your Responsibilities Include:</strong></p>
<ul>
<li>Delivering personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities.</li>
<li>Delivering dementia-informed, relationship-based care across the continuum of care.</li>
<li>Conducting comprehensive assessments and developing tailored care plans for older adults.</li>
<li>Managing chronic conditions and performing care with skill and empathy for patients and their families.</li>
<li>Collaborating with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care.</li>
<li>Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.</li>
<li>Maintaining consistency in documentation, follow-through, and patient communication.</li>
</ul>

<p><strong>What You’ll Bring</strong></p>
<ul>
<li>Active, unencumbered NP or PA license in the state you will be practicing.</li>
<li>Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).</li>
<li>Compassionate, patient-centered approach and strong clinical judgment.</li>
<li>Valid driver’s license and reliable transportation.</li>
