About the Role

Bilingual English/Spanish Required | Licensed Social Worker – Professional Counselor – Marriage and Family Therapist  The Basics  <ul> <li>Location: Orlando area (zip code 32837)  </li> <li>Salary + Bonus: $73,500 - $90,000 annually (depending on experience and bonus opportunities) PLUS $2,500 Bilingual stipend </li> <li>Schedule: 8-to-5, Monday through Friday, no charting after hours </li> <li>Reporting to: Clinical Manager </li> </ul> You'll be embedded on-site in a kidney care clinic, supporting dialysis patients who face unique mental health challenges, emotional stress, and difficulty adhering to complex treatment. As a core member of the integrated care team, alongside the dialysis team, nephrologists, and social workers, you'll see patients in person and virtually.  The Job You'll Do  <ul> <li>Conduct biopsychosocial assessments and diagnose using validated screening tools and clinical evaluation. </li> <li>Provide brief, evidence-based interventions tailored to kidney care (e.g., CBT, Motivational Interviewing, Solution-Focused Brief Therapy). </li> <li>Develop individualized care plans and review them collaboratively with nephrology and the care team. </li> <li>Participate in interdisciplinary case conferences, including consultation with a psychiatric consultant, and communicate recommendations to the team. </li> <li>Facilitate referrals to evidence-based psychosocial services when clinically indicated (e.g., grief support, psychiatric providers, medication-assisted treatment). </li> </ul>  The Right Fit  <ul> <li>Active independent Florida license (LCSW, LMHC, or LMFT): in good standing, with a Master's or higher in a clinical behavioral health field. </li> <li>Minimum two (2) years of clinical experience with patients who have co-occurring mental health, chronic medical, substance use, and physical health challenges. </li> <li>Bilingual English/Spanish speaking is required.</li> <li>Demonstrated experience supporting patients and families facing serious illness, end-of-life concerns, grief, and loss (e.g., hospice, palliative care, oncology, long-term care). </li> </ul> How We Are Different  At evolvedMD, we place clinicians on-site and in person inside the practices we serve, so you're part of a care team instead of working alone.
