Engage Senior Therapy, a proud member of <a href="https://www.fusionptpartners.com/">Fusion Physical Therapy Partners</a>, is seeking a dedicated Physical Therapist to join our team as Clinic Coordinator, delivering compassionate care at Assisted Living Facilities in Concord and Allenstown, NH. This is your chance to empower the geriatric and senior population to regain strength, mobility, and independence while working in a supportive, team-oriented environment. About the Role As Clinic Coordinator, you will lead day-to-day operations of up to two clinics, ensuring each clinics’ growth, profitability, and high-quality care delivery. You’ll mentor a clinical team while managing your own patient caseload. Responsibilities include: Patient Care <ul> <li style="font-family: verdana, geneva, sans-serif;">Provide one-on-one physical therapy sessions </li> <li style="font-family: verdana, geneva, sans-serif;">Develop individualized care plans and treat patients through all therapy phases, using evidence-based approaches to restore optimal function.</li> <li style="font-family: verdana, geneva, sans-serif;">Complete daily patient care, progress, and discharge notes.</li> <li style="font-family: verdana, geneva, sans-serif;">Attend weekly case management meetings with the care team and communicate continuously with referring physicians and referral sources.</li> <li style="font-family: verdana, geneva, sans-serif;">Ensure documentation, clinical logs, and checklists follow Engage’s policies and procedures.</li> <li style="font-family: verdana, geneva, sans-serif;">Handle patient, family member, or DPOA questions, concerns, or complaints, collaborating with internal leadership to resolve any issues.</li> </ul> Staff Management <ul> <li style="font-family: verdana, geneva, sans-serif;">Lead and supervise clinical staff (PTs, PTAs, OTs, OTAs, SLPs), ensuring high performance and a supportive work environment.</li> <li style="font-family: verdana, geneva, sans-serif;">Create and lead annual staff performance reviews, providing ongoing feedback and mentorship.</li> <li style="font-family: verdana, geneva, sans-serif;">Participate in hiring, training, and onboarding of clinical team members</li> <li style="font-family: verdana, geneva, sans-serif;">Prepare and lead staff meetings, addressing any team issues.</li> <li style="font-family: verdana, geneva, sans-serif;">Serve as a mentor or clinical instructor through Engage’s New Grad Mentorship and Student Affiliation Programs.</li> </ul> Clinic Management <ul> <li style="font-family: verdana, geneva, sans-serif;">Manage clinic schedules and ensure appropriate patient appointments in coordination with the Director of Quality Assurance.</li> <li style="font-family: verdana, geneva, sans-serif;">Prepare and submit weekly reports on KPIs, including visit rates, cancellations, and other metrics.</li> <li style="font-family: verdana, geneva, sans-serif;">Oversee the maintenance and cleanliness of the facility.</li> <li style="font-family: verdana, geneva, sans-serif;">Develop ideas for new program initiatives and assist with their implementation.</li> <li style="font-family: verdana, geneva, sans-serif;">Address any incidents involving patients, visitors, or staff, and ensure compliance with Medicare, state, and federal regulations.</li> </ul> Why Engage?
