In this role, you’ll provide direct care under the guidance of a Registered Nurse, follow the plan of care, and work closely with our interdisciplinary team to ensure each patient receives the support they need.</p> <hr data-start="1153" data-end="1156"> <h3 data-start="1158" data-end="1180"><strong data-start="1162" data-end="1180">What You’ll Do</strong></h3> <ul data-start="1182" data-end="1810"> <li data-start="1182" data-end="1262"> <p data-start="1184" data-end="1262">Provide hands-on care to patients as allowed by the State Nurse Practice Act</p> </li> <li data-start="1263" data-end="1326"> <p data-start="1265" data-end="1326">Carry out care plans based on assessments and patient needs</p> </li> <li data-start="1327" data-end="1366"> <p data-start="1329" data-end="1366">Document visits clearly and on time</p> </li> <li data-start="1367" data-end="1441"> <p data-start="1369" data-end="1441">Help educate patients and families so they feel informed and supported</p> </li> <li data-start="1442" data-end="1564"> <p data-start="1444" data-end="1564">Communicate patient needs to the wider hospice team (aides, therapists, social workers, dietitians, supervisors, etc.)</p> </li> <li data-start="1565" data-end="1619"> <p data-start="1567" data-end="1619">Use equipment and resources safely and responsibly</p> </li> <li data-start="1620" data-end="1693"> <p data-start="1622" data-end="1693">Support pain and symptom management, and monitor how patients respond</p> </li> <li data-start="1694" data-end="1759"> <p data-start="1696" data-end="1759">Participate in training, development, and quality initiatives</p> </li> <li data-start="1760" data-end="1810"> <p data-start="1762" data-end="1810">Assist with other tasks as requested by the RN</p> </li> </ul> <hr data-start="1812" data-end="1815"> <h3 data-start="1817" data-end="1847"><strong data-start="1821" data-end="1847">What We’re Looking For</strong></h3> <ul data-start="1849" data-end="2262"> <li data-start="1849" data-end="1894"> <p data-start="1851" data-end="1894">Graduate of an accredited LVN/LPN program</p> </li> <li data-start="1895" data-end="1992"> <p data-start="1897" data-end="1992">At least three years of nursing experience (hospice, community health, or med/surg is a plus)</p> </li> <li data-start="1993" data-end="2032"> <p data-start="1995" data-end="2032">Active LVN/LPN license in the state</p> </li> <li data-start="2033" data-end="2083"> <p data-start="2035" data-end="2083">Strong communication and organizational skills</p> </li> <li data-start="2084" data-end="2142"> <p data-start="2086" data-end="2142">Valid driver’s license and a reliable, insured vehicle</p> </li> <li data-start="2143" data-end="2172"> <p data-start="2145" data-end="2172">Current CPR certification</p> </li> <li data-start="2173" data-end="2262"> <p data-start="2175" data-end="2262">A genuine understanding of the hospice philosophy and the needs of the terminally ill</p> </li> </ul><div class="content-pay-transparency"><div class="pay-input"><div class="title">Hourly range for posted region</div><div class="pay-range"><span>$35</span><span class="divider">&mdash;</span><span>$45 USD</span></div></div></div>
